Definitions
- Hospice Care: Support concentrated on comfort and quality of life for terminal illness sufferers.
- Patient: An individual receiving our hospice services
- Primary Caregiver: A person designated to assist the patient with daily needs.
- Service Provider: Hospice and Palliative Care of California and its staff.Scope of Services
We offer:
- Pain and symptom management.
- Emotional and spiritual support.
- Assistance with daily activities.
- Bereavement support for families.
Our services aim to improve comfort and do not include treatments intended to cure the illness. Depending on personal needs, we offer hospice care in the patient’s house, nursing homes, hospitals, or other locations.
Eligibility for Services
To receive our services:
- A doctor has to declare that the patient has a terminal illness with six months or less of prognosis.
- Necessary medical records and documentation must be provided.
- The patient or their legal agent must agree to hospice care.
Patient and Family Responsibilities
- Provide accurate and complete health information.
- Follow the care plan as our team has advised.
- Ensure a safe environment for care, especially for home-based services.
- Appoint a primary caregiver, if necessary, to assist in daily care.
Financial Terms and Payment
- We do not bill Medicare, Medicaid, private insurance, or require self-payment.
- Policies pertaining to billing, uncovered services, refunds, or cancellations are not applicable.
Privacy and Confidentiality
- HIPAA rules guide our protection of patient data.
- Patient information is shared only with authorized people and utilized only for treatment needs.
- Patients have the right to view and ask for changes to their medical records.
Medical Consent and Advance Directives
- Before providing hospice care, we get permission from the patient or their legal agent.
- We respect living wills and advance instructions including Do Not Resuscitate (DNR) orders.
- If a patient becomes unable to make decisions, we follow the established directives or consult the appointed representative.
Termination of Services
Services may end if:
- The patient’s condition improves, and hospice care is no longer needed.
- The patient or their representative decides to stop services.
- There are safety concerns or non-compliance with the care plan.
We will assist in transitioning to other appropriate care providers if necessary.
Liability and Disclaimers
- We offer our services as stated without warranties.
- We are not liable for outcomes beyond our control.
- Patients and families consent to let us be indemnified against claims resulting from service misuse.
Grievance and Complaint Procedures
- Patients and families can file complaints by contacting us directly.
- We address concerns promptly and aim for resolution within a reasonable timeframe.
- Contact information for state or regulatory bodies overseeing hospice care is available upon request.
Amendments and Changes
- We reserve the right to update these terms.
- Families and patients will be immediately alerted of notable developments.
Governing Law and Jurisdiction
The State of California’s laws govern these terminologies. Any conflicts will be settled inside this jurisdiction by mediation, arbitration, or legal action.
